  • The Product Theft form must be completed when a parent/caregiver fails to submit the entire amount due for products received during a product program. If a parent/caregiver does not pay the troop for products received, the troop must complete this form so the troop will be released of the financial responsibility of a product theft from a parent/caregiver for the current product program. If the troop fails to complete and return this form, it is assumed the theft is owed by the troop and the troop volunteer will be held liable for the product theft. One product theft form must be completed and submitted for each girl who has any amount unpaid.

  • The following fields should be completed with contact information and details for the caregiver who signed the Parent Agreement form.

  • Three contact attempts are encouraged. It is recommended the contact attempts be made at different times to maximize the chances of making contact. Contact can be verbal (in person or phone) or written (letter, text, or email). Provide the name of the individual who made the call/contact, person contacted, date of contact, response, and comments.
