2025 Fall Product: Theft Form

The Product Theft form must be completed when a parent/caregiver fails to submit the entire amount due for products received during a product program. If a parent/caregiver does not pay the troop for products received, the troop must complete this form so the troop will be released of the financial responsibility of a product theft from a parent/caregiver for the current product program. If the troop fails to complete and return this form, it is assumed the theft is owed by the troop and the troop volunteer will be held liable for the product theft. One product theft form must be completed and submitted for each girl who has any amount unpaid.

Please submit the following required documentation with this form: Parent Agreement form, copies of signed receipts for dispersed Fall Product or Cookies, copies of signed receipts for any payments received, police report (theft-related debt), and /or insurance claim (theft-related debt). If you are missing any of the required information, you must submit what you have by the deadline, and contact Troop Banking to turn in any additional late documents.*
